  1. Must live in Utah
  2. Enter personal information in the enrollment
  3. Consultation with one of our medical providers
  4. Enrollment at Body Site patient portal
  5. Completion of enrollment through the BodySite platform (Medical History and Consent Form)
  6. After review by staff, you will be invoiced for 1-3 months’ treatment
  7. Prescriptions will be sent to the pharmacy in Salt Lake City, Utah and should be received by you within 3-5 days
  8. Body Site patient portal will give you daily tasks and instructions
